Given that you've made it this far, you should currently be at the window within the Config PPP control panel that has a dialog box asking for a telephone number and initialization string. If you followed the directions on the previous page, this information should already have been typed in.
Now, click the "Authentication..." button. You'll see a window appear that looks like this. Simply type in your username and password, hit the "OK" button and you're ready to roll!
